1846 New York en de spanningen hebben climax en fatale proporties bereikt tussen de Ierse immigranten en de Britse en Nederlandse bewoners van de Five Points-buurt. William Cutting (Day-Lewis), alias Bill the Butcher, leidt de bewoners van Five Points en creëert de 'Native Americans' bende, terwijl de immigranten worden geleid door Priest Vallon (Liam Neeson) en 'The Dead Rabbits' vormen.
Na een gewelddadige botsing tussen de twee bendes waarbij Vallon om het leven komt, wordt zijn zoon Amsterdam vermist om in 1862 onder een nieuwe naam op te duiken en wraak te nemen voor de dood van zijn vader. Amsterdam (DiCaprio) slaagt erin zich te integreren in de corrupte binnenste cirkel van Bill the Butcher, die nu de hele onderwereld rond Five Points bestuurt, en wordt zelfs zijn rechterhand. Zijn identiteit wordt echter snel onthuld en er volgt een bloedige confrontatie.
